A Nurse‑Led Postpartum Mental Health Outreach Program

We partner with OB‑GYN offices and health systems to provide structured telephonic and virtual follow‑up for postpartum patients.

Program highlights:

  • Initial call within 7 days postpartum

  • Follow‑up at 2–3 weeks

  • Virtual visit at 6 weeks

  • Optional check‑ins as needed

  • Screening tools: EPDS, GAD‑7, PHQ‑9

  • Education on emotional recovery, sleep, and identity shifts

  • Resource linkage to therapy, support groups, and community care
